2 tasks in suggested order
1. If you are an EU/EEA or Swiss citizen and plan to stay in Poland for more than 3 months, register your residence with the competent voivode.
Submit the application in person no later than the day after 3 months have elapsed since your entry into Poland.
The voivode competent for your place of stay; the case is handled by the relevant voivodeship office.
A valid identity/travel document and documents confirming the basis for residence, depending on the situation.
Registration of residence for an EU/EEA/Swiss citizen is separate from address registration (meldunek) with the municipality.

2. If you live at a specific address, fulfill the address registration obligation in accordance with the rules for permanent or temporary residence.
If the 30-day address-registration deadline applies to you, register no later than the 30th day after moving in. Temporary address registration applies when you live at a specific address for more than 3 months. Some non-EU/EFTA foreigners have a shorter 4-day deadline — use the route that matches your status.

An identity document and a document confirming legal title to the premises or confirmation from the owner, depending on the situation.
