I lost or damaged my national ID card

1. Report a lost or damaged ID card as soon as possible so that it can be invalidated. If the card was stolen, reporting the theft to the police is sufficient.
When?
Immediately upon discovering the loss, theft, or damage.
Where / who?
Online or in the mObywatel app, at any municipal office; abroad, also at a Polish consulate. You may report theft to the police.
What to prepare?
For in-person reporting: a form and—if available—another identity document; in cases of damage, the damaged ID card as well.
What to check?
Once reported, the ID card is invalidated. If you are at least 18 and report the loss of your ID card, your PESEL number is automatically restricted. The certificate of loss/damage is not an identity document and is valid until the new ID card is issued, for no more than 2 months.
