Online, at the city office (for cities with county status), a Warsaw district office, or the appropriate county office (*starostwo*).
What to prepare?
Application, current photo, proof of payment, and—if applicable—the damaged driving licence.
What to check?
A duplicate can be issued for a Polish driving licence and also for an EU-issued licence if you live in Poland. Do not drive without a valid driving licence.
