1. After changing your surname, apply for a new ID card as soon as possible.

When?

As soon as possible after the change of details. An ID card with outdated details will be invalidated 120 days after the change is recorded in the PESEL register.

What to prepare?

Current details, a photograph, and your existing document, in accordance with the procedure.

What to check?

Do not wait until the end of the 120 days if you need the document earlier — after that period, the ID card with outdated details is no longer valid.

2. If your passport details changed after marriage, apply for a new passport.

When?

The passport becomes invalid 120 days after a change of surname, first name, date of birth, sex, or PESEL number is recorded in the register.

Where / who?

A passport office in Poland or a consulate abroad.

What to prepare?

Identity document, photograph, current passport, and other required confirmations.

What to check?

When planning travel, check that the passport will remain valid for the entire required period and that the booking details match the travel document.

3. If your changed surname appears on your driving licence, apply for a document with your current details.

When?

After a change to details shown on the document, according to the current driving-licence exchange procedure.

Where / who?

The transport department responsible for your place of residence.

What to prepare?

Driving licence, identity document, photograph, and fee/payment confirmation as required by the service.

What to check?

A new national ID card does not automatically update your physical driving licence.

4. If the surname change affects the owner's details in vehicle documents or records, check the procedure for updating the vehicle data.

When?

After the change of details, if your situation requires the vehicle documents or records to be updated.

Where / who?

The vehicle registration authority.

What to prepare?

Identity document, vehicle registration certificate, and a document confirming the change of details.

What to check?

A change of surname does not automatically update your insurance policy or all vehicle registers.

5. Give your new surname to your employer or other contribution payer so they can update your details in their records and settlements.

When?

Upon receiving the document with your new details, or earlier if required by the payer.

Where / who?

Employer/HR department or your contribution payer.

What to prepare?

Document confirming the change of surname and the new details.

What to check?

6. Update your name with banks, insurance providers, utility companies, telecom providers, landlords, and other personal contracts.

When?

Once the updated document has been issued, in accordance with the provider's procedures.

Every bank, insurer, and service provider you use.

What to prepare?

New identity document and customer/contract number.

What to check?
