I am moving to Poland and want to determine my NFZ health-insurance coverage
5 tasks in suggested order
1. First identify the basis for health insurance: mandatory insurance in Poland, family-member coverage, international coordination or possibly voluntary insurance.
After arrival or when work/status changes.
NFZ / ZUS depending on the basis.
Employment/business status, family, residence and information about insurance in another country.
Do not assume that everyone moving to Poland should immediately take out voluntary insurance.

2. If you have a mandatory insurance basis in Poland, ensure correct registration by the responsible payer or fulfil the duty applicable to your category.
If applicable: If you are subject to compulsory health insurance in Poland.
From the moment the mandatory basis arises.
ZUS/NFZ through the payer or the relevant procedure.
Identification details and documents connected with the insurance basis.
Registration can differ for employees, entrepreneurs and other categories.

3. If you do not have your own insurance basis, check whether you can be registered as a family member of an insured person.
If applicable: If you can be registered as a family member of an insured person.
When you meet the statutory family-member conditions.
Registration by the insured person's payer.

Not every family relationship or situation allows registration in this form.

4. If you remain covered by another EU/EFTA public system or coordination rules, first determine the competent insurance state and the document giving access to care in Poland.
If applicable: If EU/EFTA social-security coordination rules apply to you.
Before creating a new Polish insurance basis.
NFZ and the competent foreign institution.
Documents on foreign insurance and employment/residence status.
Do not create double public insurance merely because you live in Poland.

5. If you are not mandatorily insured, cannot be registered as a family member and are not covered by another state's public system in a way that excludes this route, check voluntary NFZ insurance.
If applicable: If you have no other basis for insurance and meet the conditions for voluntary insurance.
After ruling out the previous bases.
