1. Register the child's birth at the Civil Registry Office.
When?
Within 21 days of the issuance of the birth certificate (medical birth record).
Where / who?
At the Civil Registry Office (USC) competent for the child's place of birth; if you meet the service conditions, you can also submit the notification electronically.
What to prepare?
Identity document; if acting through a proxy, a power of attorney is also required.
What to check?
After registration, the Head of the Civil Registry Office will draw up the birth certificate, assign a PESEL number, and register the child's place of residence. If you do not report the birth on time, the Civil Registry Office will register it ex officio.
