2. Check who is responsible for registering you for insurance and whether the registration was successfully completed; report any changes or deregistration through the proper channels.

When?

From the date the basis for coverage arises or ceases, within the applicable deadline.

Where / who?

Employer/ZUS, labor office, or other contribution payer.

What to prepare?

PESEL number/identity document and proof of the basis for insurance coverage.

What to check?

Usually, the contribution payer handles the registration, but you should verify the details and take action if the registration is missing.

3. If an eligible family member lacks their own basis for coverage, register them through your contribution payer; once they obtain their own basis for coverage, ensure they are deregistered.

When?

Upon meeting the eligibility criteria and following any change in the family member's status.

Where / who?

Your employer/contribution payer or ZUS.

What to prepare?

Family member's details and documents confirming the relationship or education status, if required.

What to check?

A family member's registration must not remain unupdated after they obtain their own insurance coverage.

4. If you do not have mandatory coverage or family-based coverage, check the conditions for voluntary insurance with the NFZ and then register with ZUS.

When?

Before a gap in benefit eligibility arises, or immediately after discovering one.

Where / who?

The appropriate NFZ provincial branch, followed by ZUS.

What to prepare?

An identity document, insurance history, and documents specified by the NFZ.

What to check?

A voluntary insurance agreement may entail an additional fee based on the length of the coverage gap; always check the current amount.

5. If you work, reside, or receive benefits in another country, determine the competent state and the necessary documents (e.g., S1, EHIC/EKUZ, or other confirmations).

When?

Before the cross-border situation begins or after a change occurs.

Where / who?

NFZ/ZUS and the competent institution of the other country.

What to prepare?

Details regarding employment, residence, family, and foreign insurance.

What to check?

The EHIC (EKUZ) does not replace formal registration of residence and is not a solution for every long-term stay.
